Large breasts can cause pain, improper posture, rashes, breathing problems, skeletal deformities, and low self-esteem. Breast reduction surgery is usually done to provide relief from these symptoms. Performed under general anesthesia, the two- to four-hour procedure removes fat and glandular tissue and tightens skin to produce smaller, lighter breasts that are in a healthier proportion to the rest of the body.

Breast Lift
A breast lift, or mastopexy, is performed to return youthful shape and lift to breasts that have sagged as a result of weight loss, pregnancy, loss of the skin’s natural elasticity or simply the effects of aging and gravity. The procedure can also reduce areolar size (the dark skin surrounding the nipple), and it can be combined with breast augmentation for added volume and firmness. It is usually done under general anesthesia, and lasts from one to three hours.

Breast Reconstruction
Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately following mastectomy, so that the patient wakes with a new breast mound. Alternatively, reconstruction may begin years after mastectomy. There are several ways to reconstruct the breast, both with and without implants; your surgeon(s) should work together with you in deciding which is the best for you. Breast reconstruction does not affect the recurrence of cancer, does not cause any other diseases, and does not affect chemotherapy or radiation treatment.

Nipple Reconstruction
Nipple reconstruction can provide a boost in self-confidence to patients who have inverted nipples or lack one or both nipples due to mastectomy (surgical removal of the breast), injury or a congenital defect. It is often the final step in breast reconstruction surgery, where the nipple is created after the breast mound has settled into place so that the new nipple matches the appearance and position of the natural one as closely as possible. Nipple reconstruction may be performed using one of several techniques, including tissue and cartilage flaps, skin grafts and tattooing.

Gynecomastia
Gynecomastia refers to male breast development to a larger size than what is considered as normal or acceptable. The gynecomastia could be a result of ingestion of certain medications, drugs, or steroids. Sometimes an overweight man can also have the appearance of having large breasts although they usually contain mostly fat. The cause of this condition should be thoroughly investigated by an endocrinologist before it is treated. If the cause is medication those could be modified and if it is drug or steroid related and the person continues to use these then treatment will not result in a cure. Some males not only have large breast tissue but also a large areola. Some have large amount of extra skin and stretch marks as well. During treatment of this condition if any significant breast tissue is present it should be sent to pathology. Males could develop breast cancer just like women but of course it is my less common. Teenagers should not be concerned with breast cancer,however, older men should.
Gynecomastia can be treated with suction only as there are special cannulaes to address the fibrous tissue that usually is present. Sometimes, however, the tissue is so dense that an incision must be made around the areola to remove it. If a lot of extra skin is present with or without a large areola, different types of skin reduction procedures must be done depending on the amount of skin that must be removed. Needless to say that if a skin reduction procedure is necessary, one always hopes that there is enough hair on the chest to hide the resultant scars. Gynecomastia surgery can be done as an outpatient procedure under sedation and local anesthetic. Antibiotics and pain medications are given upon discharge. Results are often very good.
If you have such a condition it is best to be checked by your internist or pediatrician first to rule out any underlying pathology. If there is no underlying cause then a plastic surgeon is best consulted.
